We participate in most major medical plans including Medicare. Your insurance is a contract between you, your employer and the insurance company. Payment for your services is ultimately your responsibility. It is therefore important that you verify provider participation and benefits with your insurance company prior to your visit. If we are a contracted provider, we will file a claim on your behalf.
However, we are NOT in network with CareSource, Molina, United Healthcare Community Plan and Worker’s Compensation. Please call the office if you have any questions as to whether we are an in-network provider or not.
High Deductibles/Cash Only Procedures
Payment is due at the time of service for any amounts known to be not covered or paid by your insurance plan. This includes all copayments, coinsurances and deductibles. For cash only procedures performed at the hospital, surgery center, or office, payment is due 1 week before the scheduled procedure.
New Patient Packet
If you would like us to be able to obtain your medical records from your previous physician, please fill out the Medical Release Form so we can send this to your previous physician to obtain your records.
